Upon scheduling at appointment at Elevation Dermatology you will receive an email with a link to our patient portal. Feel free to complete all of the new patient intake forms within the portal in the convenience of your own home or office prior to your first appointment. The “Release of Records” form below may be printed and completed so that we may obtain any other past medical records from outside facilities.
